(1) I, hereby authorize Reinvent U Boot Camps to withdraw the fee of $59.00 via automatic credit/debit (information provided above) on a monthly (every 30 days) from the original date of this agreement for group fitness membership.

 

(2) Reinvent U Boot Camps will be listed on your bank statement, which will constitute receipt for payment on your account.

 

(3) Payments will automatically be renewed after your agreed time unless a 30-day written notice is provided to Reinvent U Boot Camps frances@reinventuspawellness.com.

 

(4) The privilege of making payments under the arrangement may be revoked by Reinvent U Boot Camps if any items are not paid upon presentation.

 

(5) If this auto debit payment arrangement is revoked for any reason, it does not release you from your financial obligation.

(6) A service fee of $50.00 will be assessed and drafted from your credit/debit card on file for insufficient funds.

 

(7) Client payments are non-transferable and non-refundable.

 

(8) Company does not provide Clients with credit, or a monetary reimbursement should you choose or cannot attend your group fitness classes at Reinvent U Spa & Wellness.

By signing below, you are certifying that you are at least 18 years old and that you have reviewed and agree to the Terms and Conditions of the Agreement. You further understand that you are responsible for payments described herein. Acknowledge and Agreed.
